Cost of Future Care

A Cost of Future Care Analysis is a comprehensive report which sets out items, goods and services which have been identified as being reasonably necessary to attempt to restore the injured client back to the position he or she would have been in had the injury never occurred. The objective of such an analysis is to identify those items and services which may be required to maintain his or her physical, cognitive and psychological health, maximize his or her independence and safety, and replicate as closely as possible the lifestyle he or she would have experienced had the injury or illness not occurred. An analysis of the individual’s status and function prior to and subsequent to the injury or illness sets the stage for a determination of anticipated and related costs. A Cost of Future Care report lays the foundation for disability management throughout a person’s lifetime.

Rehabilitation Case Management

Case management is a collaborative, client-driven process for the provision of quality health and support services through the effective and efficient use of resources. Case management supports the client’s achievement of safe, realistic and reasonable goals within a complex health, social and fiscal environment. The case management process that starts with an initial comprehensive physical and functional assessment and report. Contained in the report are treatment recommendations, including regionally appropriate treatment and/or service providers.

Working closely with the physician, patient, family members and other health care professionals, the case manager looks for highly effective, outcome-based rehabilitation facilities. As treatment goals to improve health and physical functioning are reached, the focus of case management shifts to returning an individual to a productive work life where possible or appropriate. The rehabilitation case manager evaluates return-to-work potential, with a primary goal of getting disabled workers back to the same job.

Functional Capacity/Abilities Evaluation

A Functional Capacity Evaluation (FCE), also known as a Functional Abilities Evaluation, is a comprehensive test procedure designed to determine the examinee’s current capacity to dependably and sustainably meet the physical demands of a specific occupation or alternative occupations. The FCE also assists in determination of the examinee’s capacity to meet the physical demands of domestic life and recreational activity.

Testing is typically completed in one day over a 6-8 hour period. Key elements of the FCE are the determination of the level of physical effort produced by the examinee and the analysis of the examinee’s reliability in reporting pain and disability.

At the conclusion of the assessment the evaluator provides a comprehensive report documenting the findings and observations, the examinee’s subjective reports, and an analysis of the examinee’s abilities and limitations pertaining to work, household, and leisure activities.
